Opium Sands is most known for his mysterious nature. He has a rather highly developed habit of always lurking at the edges cloaked in shadow, listening unobtrusively to the usual things on that are transpiring wherever he happens to be, and then deciding upon courses of action to take based upon his findings. Although, this can easily be seen as a somewhat plotting and scheming nature, Opium has no interest in royal ambition or to dethrone the Red Queen at all. Rather, his mind is always working, and he's quite sharp when it comes to deciphering when people are attempting to lure or lie to him. Naturally a very suspicious and distrustful person, he always searches for half-truths and signs of trickery. Though he always appears calm and dignified on the outside, for someone so civilized and cultured, he is nearly heartless and even, one might say, merciless. The warning signs are always obvious with him - a substantial narrowing of the eyes, followed by an impatient twitch of the lip, and then a downright scowl, are all markers of his gradual descent into rage. He's even been known to do the Red Queen's dirty work and use his tools for murder. Almost charming in public, one should be wary of his smooth talking jibes and sarcastic wittiness. His essentially transparent passive front doesn't do a particularly good job of concealing his true, dangerous tendencies, though.
Based on these extreme aspects of his personality, Opium cannot be called the sanest person in the world. Intriguingly, however, he actually has very powerful views on madness. He is constantly searching for the real, deep meanings of life, what makes others “tick”, and considers himself to be the sole person that must solve the mystery. He simply adores women, though only attractive ones, obviously. He tends to like the more clever and feisty ones, as they are more of a challenge in obtaining, or also the more provocative ones. But despite his primal urges to possess some of them, they are objects to him, and selecting fine women is like selecting and being knowledgeable of fine wine. In all of his quirks one would question his desire to pursue a doctor's field. Opium a doctor? Quite right. He is well versed in shirking potions and growing cakes and all sorts of mundane arts. In the eyes of the world, he hides all hints of cruelty and obsessiveness, instead playing this role, dealing to wounds and sickness wherever he is called to; for a small fee.

Opium is a tall man of a lean, wiry shape with narrow, but strong shoulders and an overall skinny body. Wearing his plain black shoes he stands about 6’4 at the shoulders. His head is covered with short unkempt ruddy brown hair. He is often seen wearing a large crimson trench coat with a pair of wore black pants peeking underneath. The coat, itself, is decorated and ornate in the front, thin metal slots carved in swirling designs that can close off his chest. His skin is pale, but not so pale that he is mistaken for the walking dead. Adorned with high cheekbones, he has a lean face containing a gaunt look under his eyes and facial features from lack of sleep from his work.

- Being a doctor well versed in medicine, Opium has even perfected his own brand of poison, one of the most deadly in Underland, and is a well kept secret to all those still alive who haven't yet tasted it. Years of experimentation upon live subjects have led him to his findings, concocting a toxin strong enough to kill in about an hour, yet it is extremely painful and will burn the skin from the inside, out. It is made out of Bandersnatch saliva and a mixture of poisonous berries and powders. The only known cure is in the doctor's hands and good luck trying to get him to give you the antidote if Opium had reason enough to poison someone in the first place.
- Knowing that he would have to build up an immunity to a number of poisons including his own, he injects himself with a small does poison into the veins of his lower arms. The thin scars of the needles he hides underneath jacket. Needless to say, his body is highly resistant to almost all poisons known to Underland.
- Opium is fond of his own methods. He sometimes injects a large quantity toxin inside of him, just to experience the delightful sensation it leaves his body afterwords - even if it does occasionally make him sick.

Opium Sands was the eldest son born to Lady Annabella and Lord Theniun with his younger brother Tristen, as twins. Growing up, he always proved not to be the hero in shining armor that his parents always wished him to be. From an early age, he avoided confrontation, and when he couldn't help but get into a scrap, he did it using his brain – and using dirty means to achieve victory. His father was general (who married one of the queen's daughters) by trade, and was a close companion with dealing with the previous Queen of Underland at the time. As a child, he was introduced to Iracebeth and Mirana during a short few series of playdates, but once. It was then and only then he favored Mirana over Iracebeth, which led to a few jealous fits from his cousin. Now, he had changed his tune, simply because Mirana has become weakhearted. As eldest and heir to the family fortune, as being an offshot of royalty, Opium was expected to take up the sword and train under his father and general in the arts of war and politics like his brother. At first, he grudgingly complied, yet always preferred in using different ways to get what he wanted instead of charging forward like a blood craved idiot. He was forced to learn, and in time, became moderately proficient with a blade. Of course, he didn't hold a candle to his brother, who became quite the swordsman in their youth.
The time came when his brother was sent off to fight in a real battle, confront a skirmishing bunch of northern rabble in the field. Tristen took to the news like a fish to water, and while Opium never quite understood his brother's passion, he was anxious for his safety. When his brother went left, Opium stole away with him secretly and by night, wanting to make sure Tristen wouldn't get harmed or killed in the battle. This continued for some time, traveling close by and making a living on the road without a specific trade – a sell sword here for a few coins there. When time came for battle, he hid behind enemy lines, watching from the shadows. One day, like any other day, he followed his brother into battle after hearing wind in a local village not far off. When he arrived, Tristen was injured. Acting fast, he ran to his brother's side but after not knowing how to save him, his brother died in his arms. Bitter and jaded, Opium left his brother, swearing to his grave that he would stop learning how to perfect his sword talents in honor of preserving his brother's name, learning the things he could have done to save his brother. After this incident he returned home after a year, now seventeen. Upon his return he learned that his father had been murdered by a rebel rousing riot in one of the outskirts. He remained alone with his mother, until she reintroduced him to his aunt and cousins for the remainder of his teenage years. Opium made good his oath, and after years of studying elixers of shirking and growing, and mending wounds with bite sized mushroom caps and potions, Opium has become the doctor he always wanted to be. In recent days, his mother has passed away, and even though he is still bitter towards the destruction of his family, his heart is slowly mending under its weight. When Iracebeth took power, he turned away form former favorite cousin Mirana, instead looking towards Iracebeth for guidance. He has sworn his loyalty to her (as of now), hoping that she can heal him and push aside the guilt and emotions that threaten to turn him into a monster.
